It is the first expansive web-based brain health registry in the Commonwealth. The National POLST form is now approved for use in Virginia. While the Virginia POST form may still be used, transitioning to the National POLST form is recommended.
This advance directive (“AD”) complies with the Virginia Healthcare Decisions Act. You are not required to use this form to create an AD.
